Terms of Service
Effective Date: July 1, 2025
These Terms of Service ("Terms") govern your access to and use of the website and telehealth services provided by Mental Well-Being Solutions ("Dr. Adeyemi Olubambo, DNP, PMHNP-BC"). By accessing our website or scheduling or receiving services, you agree to be bound by these Terms. If you do not agree, please do not use our services.
1. Services Provided
Mental Well-Being Solutions provides outpatient telehealth psychiatric services, including psychiatric evaluation, diagnosis, medication management, and follow-up care, via secure video platform to eligible patients in the state of Arizona.
Our services are intended for adults (18 years and older) unless a parent or legal guardian provides written consent for a minor. We do not provide emergency psychiatric services. If you are experiencing a psychiatric emergency, please call 911 or go to your nearest emergency room. For mental health crisis support, call or text 988 (Suicide and Crisis Lifeline), available 24/7.
2. Telehealth Consent
By scheduling and attending a telehealth appointment, you acknowledge and consent to the following:
- Telehealth involves the delivery of healthcare services using electronic communications and may include video, audio, and electronic transmission of medical information.
- You understand the potential risks of telehealth, including technical failures, limitations in physical examination, and the possibility that information transmitted may be insufficient for clinical decision-making.
- You have the right to withdraw consent for telehealth services at any time and request in-person care.
- Telehealth services are subject to the same standards of care as in-person services.
- You will be located in the state of Arizona during all telehealth appointments, unless otherwise agreed in writing.
3. Patient Responsibilities
As a patient, you agree to:
- Provide accurate, complete, and current information about your health history, medications, and symptoms.
- Notify us promptly of any changes to your health status, medications, or contact information.
- Attend scheduled appointments on time or provide at least 24 hours' notice for cancellations.
- Use our services only for lawful purposes and in accordance with these Terms.
- Not share your account credentials with any other person.
- Ensure a private, distraction-free environment during telehealth sessions.
- Understand that prescriptions are issued at the sole clinical discretion of the provider and are not guaranteed.
4. Appointments and Cancellations
Appointments must be scheduled in advance through our online booking system or by contacting our office. We strive to offer same-day and next-day availability.
Cancellation Policy: We require at least 24 hours' notice to cancel or reschedule an appointment. Late cancellations (less than 24 hours' notice) or no-shows may be subject to a cancellation fee. Repeated no-shows may result in discharge from the practice.
We reserve the right to reschedule or cancel appointments due to provider illness, emergency, or circumstances beyond our control. We will make reasonable efforts to notify you as soon as possible in such cases.
5. Fees, Billing, and Insurance
Fees for services are due at the time of service unless other arrangements have been made in advance. We accept most major insurance plans. It is your responsibility to verify your insurance coverage and benefits prior to your appointment.
You are responsible for any co-pays, deductibles, co-insurance, or non-covered services as determined by your insurance plan. If we are unable to collect payment from your insurer, you agree to be responsible for the full cost of services rendered.
Self-pay rates are available. Please contact our office for current fee schedules.
6. Prescriptions and Controlled Substances
Prescriptions, including controlled substances, are issued solely at the clinical discretion of the provider based on a thorough evaluation. We comply with all applicable federal and state laws governing the prescribing of controlled substances via telehealth, including the Ryan Haight Online Pharmacy Consumer Protection Act and applicable DEA regulations.
Patients prescribed controlled substances may be required to sign a controlled substance agreement, submit to random urine drug screens, and comply with prescription monitoring program requirements. Misuse, diversion, or violation of the controlled substance agreement may result in immediate termination of services.
7. Limitation of Services
Our telehealth services are not a substitute for emergency care. We do not provide:
- Emergency psychiatric services or crisis intervention
- Inpatient or residential psychiatric treatment
- Forensic psychiatric evaluations
- Services to patients who are acutely suicidal, homicidal, or in need of immediate hospitalization
If at any time you are in crisis, please call 911, go to your nearest emergency room, or call/text 988.
